Please fill out the required fields below to request an official Permission to Tryout (PTT) form through the Ontario Women’s Hockey Association (OWHA).
A Permission to Tryout form is required for any player wishing to try-out for another female hockey association.
Once your request has been received, our registrar will review your submission and provide you with an authorized PTT form, which must be presented at the association where you intend to try-out. Please allow up to 72 hours for processing and delivery to the email address provided below.
**Permission to Tryout forms are valid as of 12:01 am on April 14, 2026 to May 31, 2026 and from August 14, 2026 until a date to be announced by the OWHA.
