Player / Team Staff / Volunteer Vaccination Submission Info
IMPORTANT:  Players (2009 & older), ALL coach  / bench staff and volunteers

As per the OWHA requirements, please click on the link below, fill out all information and upload the applicable proof of vaccine receipt for ALL required players, team staff and volunteers.

This submission will only be visible by the representative receiving these. This is needed this year by October 17th, 2021.

Please refer to the OWHA vaccine policy for further information on the policy if needed.
