Permission to Skate forms:
A permission to skate form is required to try out for any organization other than the one you were registered with for the 2021-2022 season.
Permission to skate forms are NOT a player release. Player Release forms are done separately
If you require permission to skate form from Lady Flags please email: Marilyn Maoirat at
[email protected] with the following information:
- Player Name
- Team and organization that player is trying out for
If you are choosing to try out for multiple teams/organizations, you will require a separate form for each
Once Marilyn has received your email, the form(s) will be prepared for pickup. You will receive an email indicating when the form(s) are ready for pickup.
