This form must be used when a player wishes to attend a tryout hosted by another OWHA association or OWHA independent team. A player must receive a Permission to Tryout or a release from her last registered OWHA association or OWHA association team or in order to be eligible.
Please complete the questions below to be signed by a designated executive member of the player's current association/team and presented to the association/team for which the player wishes to try out before she is allowed on the ice.
